Is this is your first session with Amanda Lynne?
Or if you have had longer than 12 months between sessions with Amanda Lynne?
If so, you will need to complete the attached consult record.
NOTE: This is a 4-page intake form and needs some time and contemplation to complete. Therefore, please complete and return this by email prior to your session so I may print it in readiness for your session, alternatively complete and bring a printed version to your in-person session.
Alternatively, if you are attending an In-Person session, you may arrive to the Day Spa least 30 minutes prior to your consult and complete it in the waiting room.
There will not be time during your session to complete the intake session.
Upon completion of your client record:
Please email it to Amanda Lynne at least 2 hours prior to your session taking place, alternatively if you are attending an in-person consult, you may also bring it on the day of your booking.
Please note without your client record, your session cannot go ahead and you will need to reschedule.
